Diabetes RN Educator: 30 hours bi-weekly/ 7:30a-3:30p Part Time

|Description:||The diabetes educator partners with individuals and their supportive team affected by diabetes or in danger of developing diabetes, to learn about the disease and develop the skills they need to manage it successfully by modifying their own behavior. The diabetes educator has in-depth knowledge of biology and social sciences along with being skilled in communication and counseling. The education process includes but is not limited to teaching the client about diabetes, nutrition, appropriate activity levels, glucose monitoring, medication and other diabetes care devices. The diabetes educator is an integral part of a multidisciplinary team which provides diabetes education, self-management training, facilitate continuity of care which results in client goal attainment and optimal health. Coordinates and completes required documents and activities necessary for maintaining recognition from the American Diabetes Association. Works closely with finance, nursing and public relations department to ensure program viability through efficient scheduling practice, timely billing, patient care coordination and marketing. |
|Qualifications:||A registered nurse must hold a current, active and unrestricted NYS license. (A 4-year degree is preferred but will accept others with comparable experience) A registered dietitian must hold active registration with the Commission on Dietetic Registration and an unrestricted NYS license. Certification in Diabetes Education is preferred however, applicants who qualify for certification eligibility will be considered. Must have 2 years work experience in their work profession and must meet education standards of 15 hours of Continuing Education yearly as required by the American Diabetes Association for program recognition. Insulin pump management/certification a plus.|
